Required Qualifications:
QUALIFICATIONS:
• Minimum diploma in Social Service Work, or a related field
• Preference for a diploma in Assaulted Women’s and Children’s Counsellor studies or a degree in Social Work
• Minimum two (2) years work experience in any of the following fields: addiction, mental health disorders, concurrent disorders, counselling and other social services supporting vulnerable women
• Preference for two (2) to four (4) years of experience working in the above fields
• One (1) to two (2) years related experience in community outreach and program promotion
• Experience developing and delivering group supports to women with varying needs
• Excellent crisis intervention skills
• Ability to work independently and as part of a team
• Excellent organizational skills
• Committed to upholding the organization’s mission, values and standards
• Fluency in French is an asset

Key Responsibilities:
POSITION SUMMARY:
The Transitional Housing Support Worker (THSW) uses a client-centered approach to support women-identified individuals and their dependents in finding and maintaining housing, and connecting with community resources to support in their transition into housing and/or out of shelter. The Transitional Housing Support Worker supports residential and outreach service users in developing a transitional support plan and safety plan.
KEY RESPONSIBILITIES:
• Support the development of a transitional support plan and a safety plan for women and their dependents that have experienced gender-based violence or abuse, including, short-term and long-term goal setting which may involve: safety planning and risk management, finding and maintaining housing, applying for social assistance, applying for social housing, securing financial assistance, connecting with counselling and other support services, connecting with community resources, as requested by the service user
• Support clients in Women’s Services transitional housing units in on site weekly meetings, including housing support, programming, and community referrals
• Provide follow up to service users at three (3) and six (6) months after program discharge
• Provide information on resources to service users and referrals as identified by service users
• Keep systematic records using automated case management software and in-house tools developed by Inasmuch House
• Identify child welfare concerns and comply with Duty to Report obligations in accordance with the Child and Family Services Act
• Represent the THSW program in the community through public education and promotion.; this will be done in consultation with the manager of outreach and shelter operations and partner agencies
• Participate in outreach department meetings
• Participate in weekly program meetings with the manager of outreach and shelter operations
• Participate in monthly Hamilton THSW meetings
• Participate in yearly regional THSW meetings
• Participate in regularly scheduled staff meetings and trainings
• Work safely, identify any health and safety concerns and report to management, and follow Mission Services workplace health and safety policies and procedures at all times
• Ensure that service users have access to participate in barrier-free services, assist and empower each service user to become more independent
• Adhere to a strict confidentiality policy
• Other duties as required to ensure the continuation of service to residents

HOURS OF WORK:
• 37.5 hours per week
• Mainly day shifts, with the possibility of also working evenings and weekends depending on program needs
