Job Position: Community Health Worker

Required Qualifications:

. Access to a reliable vehicle and a valid driver’s license (G or G2)
. A willingness to work every other weekend
. Support Bayshore’s commitment to a high quality, safe, environment
. Ability to pass or satisfy a Criminal Background screening upon hire
. Ability to satisfy a reference check
. A passion for making a difference in your community
. Live in the community you will be servicing

Key Responsibilities:

. With an assigned client caseload, the CHW travels to clients’ home to provide the required assistance.
. Personal care – Assisting with bathing, showering, toileting
. Dressing and putting on shoes, brushing hair, teeth, shaving
. Assisting with use of walkers and canes, transfers from bed, chair, wheelchair, toilet
. Meal planning, safe food preparation and storage, feeding
. Basic home maintenance and housekeeping
. Socializing and companionship for those in need of emotional support
. CHWs may see up to 10-12 clients in a day

others in need (children, seniors, neighbors, etc.)

from Retail, Hospitality, or the Service Industries!

This position currently qualifies for the $3.00 per hour wage enhancement after your training has been completed.

Community Health Workers (CHWs) support clients in the community to live independently in their own home.
